Job Scope:
This position coordinates and provides clinical oversight to the mental health rehabilitation program and group services for adults living with serious mental illness, as well as providing a fully array of direct service to a partial caseload of clients.
Develop and lead programming that provides group and individual skills training that targets areas of functional impairment and promotes independence through the development and use of natural community supports.
Effectively engage consumers in treatment planning that fosters hopefulness and motivates change.
Participate as part of department leadership team, provide program orientation, monitor staff documentation, complete payroll and lead team meetings.
Proof of COVID-19 vaccination required.

Program ResponsibilitiesCoordinate day-to-day clinical and administrative operations and activities of mental health group programming for BH Recovery services, including the Psychosocial Rehabilitation (PSR) Program to ensure it meets HFS/Medicaid program policies and Rule 132/140 requirementsWith input from Manager and other group counselors, create and update PSR and Community group schedules that assure mental health rehabilitation and recovery oriented programming, explain group offerings and answer questions from consumers related to the schedule.
Receive referrals for PSR individual and group services and assure timely assignment to teamAct as primary liaison for referrals from KYC intake, and KYC's intensive community-based teams and grant programs (ex.
Front Door, Comprehensive Transition Program, Permanent Supportive Housing) and provide assertive outreach and creative engagement into group servicesProvide weekly scheduled and ad-hoc clinical and administrative supervision to a team of 4-8 direct reports, including clinical direction and assistance with IATP and service deliveryReview and oversee IATP for clients attending PSR to facilitate LPHA signature (sign directly for LPHA staff, or refer to assigned LPHA)Monitor individual productivity goals and oversee performance to ensure team meets goals and all activities are compliant with standards, policies and procedures of KYCCoordinate with team and authorization specialist to ensure pre-authorizations for services are submitted for all clientRegularly attend team meetings for other agency BH programs to educate on group service offerings, and discuss potential referralsOversee and ensure completion of group services administrative dutiesParticipate with manager in monitoring team staffing needs and hiring and orientation of new staffCoordinate with Manager to lead/co-lead team meetingsCommunicate with transportation facilitator and environment of care team to assure access and maintenance issues are addressedLead/co-lead groups as needed to provide coverage and staff trainingIndividual ResponsibilitiesProvide a full range of clinical care to a reduced caseload of clients, including assessment, treatment planning, outreach and therapy servicesAccurately complete clinical documentation including bi-annual assessments and treatment plans and daily service documentation and submit timely paperwork per KYC policyParticipate in weekly clinical supervision; seek consultation when neededMeet and exceed individual productivity targetProvide case coordination and linkage services to adults referred from the state operated facility living in KYC service areaRoutinely consult with other Team members including employment, transitional living, MISA/SUPR, Team Services, Dropin, and psychiatry to address specific client related clinical issues and coordinate care strategiesParticipate in orientations, all staff trainings, and inter-department meetingsInterpret and communicate, at the service delivery level, Center policies and proceduresSubmit, in a timely and accurate fashion, schedules timesheets, mileage and other reports required by the Center's policies and proceduresEducation and ExperienceMaster's Degree from an accredited college or university in social work, psychology, or counselingPreferred Illinois LPHA (as LCPC, LMFT, or LCSW) or eligible for licensure in next 12 months.
Required supervisory, leadership, and/or program development experience.
At least 1 year experience providing a variety of treatment interventions to adults with serious and persistent mental illnessBasic computer and smartphone proficiency, knowledge of Word, Excel, strong typing skills, and internet skills; ability to learn and use team collaboration tools (ex.
